Candidiasis, as a rule, develops in immunocompromised patients. Cellular immunity plays a leading role in the body's immune defense. It helps localize the infection, activate other resistance factors and destroy the pathogen. In this case, the main protective function is performed by T-lymphocytes - effectors of delayed hypersensitivity CD8 + and CD4 + type 1 T-cells. Humoral immune response to Candida spp. has its own characteristics associated with the structure of fungi and their wide distribution in the environment. Failure of the suppressor function of the immune system can contribute to the development of immunopathological reactions.

The most important role of nonspecific factors, in particular the phagocytic system, in the body’s resistance to Candida spp. At the same time, insufficiently effective elimination of the fungus in the early stages of infection involves immune defense mechanisms, which also have an important protective function. When mice are infected Candida albicans in animals pre-immunized with a sublethal dose of the same fungus, the contamination of organs with the pathogen (the number of cells in the affected organs) and mortality are significantly lower than in non-immunized mice.

Among the immune mechanisms, the most important role in protection against fungi of the genus Candida has a cellular component of immunity. Administration of antiserum against thymocytes more than doubles the lethality of mice during experimental candidiasis. There is evidence that the adaptive transfer of T lymphocytes (Tl) sensitized to C. albicans, protects linear mice from candidiasis. In humans, congenital or acquired pathology of the thymus gland is usually accompanied by the most severe forms of candidiasis.

The method of infection by the fungus has a significant influence on the development of the cellular immune response. Experiments on mice showed that subcutaneous administration of a viable culture of C. albicans induces the development of a pronounced delayed-type hypersensitivity reaction (DTH), and the period of complete elimination of the pathogen from the body coincides with the time of maximum development of the cellular reaction. In this case, the primary slow-type reaction to Candida develops on the 4th day after antigen administration, reaches a maximum on the 28th day with a further gradual decrease in the intensity of the reaction until the 75th day (observation period). A secondary cellular immune response is detected within 24-48 hours. In contrast, intravenous administration to mice C. albicans causes only mild HRT and does not alleviate the course of systemic candidiasis. People do not develop lasting immunity to Candida spp. There are two main ways of destruction of the fungal antigen during HRT: 1 - destruction of cells infected with the fungus, with the direct participation of CD8 + T cells, which recognize pathogen antigens on the affected cells in combination with MHC (major histocompatibility complex) type I antigens and release granules containing cytotoxic proteins (perforin, granzyme, etc.) that disrupt the integrity of the cell wall and induce the death of the affected cell; 2 - activation of macrophages by T helper cells (Tx) type 1 CD4 + with the participation of IL-2, gamma interferon, which enhances the death of fungi in the phagolysosome of the macrophage.

The T CD8+ effectors of HRT are of particular importance in immune protection against candidiasis. After depletion of this subpopulation, Tl show greater sensitivity of animals to systemic candidiasis than with depletion of CD4 + cells; At the same time, genetically determined characteristics of the functioning of CD4 + T cells are associated with different susceptibility to experimental candidiasis of mice of the BALB/c (highly resistant) and CBA/CaH (lowly resistant) lines. In humans, the relationship between the development of candidiasis and the severity of the decrease in CD4 + T cells in acquired immunodeficiency syndrome (AIDS) is known. Cases of candidiasis have been described against the background of idiopathic T-lymphocytopenia, characterized by a selective decrease in the number of CD4 + T cells to a level lower than 20% of the total number of T cells.

Among subpopulations of CD4+ T cells, T helper (Th) type 1 cells play a protective role in candidiasis, while activation of Th2 usually has a negative effect on the course of the infection. Thus, administration of glycyrrhizin (GH mice) to mice, leading to suppression of the synthesis of type 2 cytokines, significantly increases the survival of animals with systemic candidiasis. Inoculation of T6S cells (type 2 Tl clone) into mice abolishes the resistance of GR mice to Candida. Resistance to S. albicans can be caused by injecting animals with monoclonal antibodies against type 2 cytokines (IL-4, IL-10). In 46-81% of patients with persistent candiduria (with and without candidemia), an increase in the concentration of IL-4 and IL-10 in the blood serum was noted.

The importance of cellular immunity in protecting individual organs and systems from Candida spp. not the same. The systemic response helps localize the infection, but does not provide sufficient protection for the central nervous system and mucous membranes. In experimental disseminated candidiasis in mice immunized with S. albicans, T cells appear in the brain tissue later and in smaller numbers, and the process of eliminating the fungus is 2.5 times longer compared to the liver. When CD4 + and CD8 + T cells are depleted, the rate of liver cleansing decreases, but this does not affect the contamination of brain tissue by the fungus.

In natural routes of infection, local T cells perform an important protective function in the early stages of infection. Mice lacking γ/β T cells (located in the skin and mucous membranes and capable of recognizing antigen without prior processing and presentation by antigen-presenting cells) are highly susceptible to disseminated candidiasis of endogenous origin, while the deficiency of T cells having α/β- chain (Tl participants in the systemic T-cell reaction), increases the sensitivity of animals to infection caused by intravenous infection with C. albicans .

In the different resistance of individual organs to Candida importance is attached to the characteristics of the reaction of polymorphonuclear leukocytes (neutrophils) (PMN). In BALB/C mice, early (in the first 1-8 hours after infection) and late (after 24-48 h) activation of neutrophil-kβ. In susceptible organs (kidneys), early activation of neutrophils was not observed. One of the key molecules involved in the early activation of neutrophil-kβ is PAF (platelet activating factor). In all organs studied, the kinetics of neutrophil activation correlated with the level of mRNA expression of the proinflammatory cytokine TNF-α (tumor necrosis factor), which is involved in stimulating the development of an immune response along the T-pathway type 1.

Violations of the cellular component of immunity occur in various forms of candidiasis.

We observed a relationship between the development of the cellular reaction and the duration and extent of the pathological process. Among patients with various forms of candidiasis (acute and chronic candidal vulvovaginitis, candidiasis of the gastrointestinal tract, combined candidiasis of the gastrointestinal tract and genitals), most often - in 48.6% of cases - positive cellular reactions to the antigen Candida occurred in vulvovaginal candidiasis with a disease duration of up to 3 years, less often (in 33.8% of cases) - in women with a longer course of the disease. With a combination of gastrointestinal and genital candidiasis, positive reactions were detected in only 11.7% of patients.

With particular consistency, defects in T-cell immunity are noted in the most severe form of chronic candidiasis - chronic candidiasis of the skin and mucous membranes (CCMS). To develop HRT to Candida Attempts have been made to use the transfer factor. It has the ability to stimulate the synthesis of surface receptors of “narrow specificity” on T lymphocytes and restore their functional activity towards a specific antigen. In experiments on animals (guinea pigs, mice), this protein, as a rule, inhibited the migration of leukocytes in vitro and stimulated the development of HRT to the fungal antigen. The use of transfer factor in the treatment of patients with CCCS in a number of cases also contributed to improving the clinical course of the disease and prolonging the period of remission, however, in most patients the treatment was ineffective, which reflects the ambiguity of the mechanisms of development of immunosuppression in candidiasis.

According to various authors, insufficiency of T-cell immunity may be due primarily to a decrease in the expression of T-lymphocyte (TL) receptors, increased apoptosis of the latter, impaired cytokine synthesis, and immunomodulatory properties Candida, as well as a combination of these factors.

A decrease in the number of lymphocytes expressing E-receptors (CD2 - glycoprotein molecules that characterize the maturity of T-cells and are involved in their activation) was detected in the majority of the examined patients with CCCS.

It is known that the regulation of functional cellular activity, including the expression of receptors on the surface of T-lymphocytes, is determined by the balance of intracellular cyclic nucleotides adenosine monophosphate (cAMP) and guanosine monophosphate (cGMP). In CCCS, in 70-75% of cases, an inverse correlation was noted between the amount of intracellular cAMP and E-receptors on Tl. In experimental estrogen-dependent vulvovaginal candidiasis in mice, impaired expression of α integrins by T cells was revealed - 4, β-7, α-M290 β-7 and α-4 β-1, which negatively affects the penetration of immune T cells into the lesion.

It was found that peripheral blood lymphocytes of patients with CCCS exhibit increased apoptosis when exposed to antigens Candida. This fact may be important in the pathogenesis of candidiasis. Mice genetically deficient in Fas molecules (apoptotic molecules) on T cells synthesized significantly more proinflammatory cytokines, and the mortality of Fas-negative mice from disseminated candidiasis was significantly reduced.

In a number of patients with candidiasis caused by S. albi-cans depression of cellular immunity is caused by the presence of the pathogen in the body, while elimination of the fungus leads to the restoration of HRT to specific and nonspecific (sheep red blood cells, tuberculin, etc.) antigens. It is believed that the inactivation of the Fc fragment is caused by proteinase C. albicans This effect may be one of the reasons for the disruption of the absorption activity of phagocytes, in particular, neutrophils, found in the majority of patients with CCCS in autologous blood sera and absent in the AB(0) serum of healthy donors. Recently, the importance of individual cytokines in the regulation of the body's resistance to candidiasis has been intensively studied. Studies are usually conducted in animal experiments using approaches based on neutralization of cytokines with appropriate antibodies, or using “knockout” mice lacking certain cytokine genes or cytokine receptor genes, as well as transgenic mice (having a corresponding additional “transgenic” gene). To date, the influence of many cytokines on resistance to Candida, The protective role of proinflammatory cytokines has been established.

Particular importance is attached to cytokines that affect the function of PMNs as one of the main lines of defense against Candida. Neutralization of tumor necrosis factor TNF-α by anti-TNF-α antibodies promotes the rapid development of disseminated candidiasis and significantly aggravates the course of systemic candidiasis. Infection with S. albicans mice TNF -/- LT (lymphotoxin) -/- (double knockout) leads to a 10-fold increase in the contamination of animal organs with the fungus, a 1000-fold increase in the germination of the latter into organs, a significant increase in animal mortality compared to the control TNF +/+ LT + /+ group. TNF -/- LT -/- mice showed a pronounced delay in the renewal of the neutrophil pool in areas of accumulation Candida, decreased phagocyte activity, decreased synthesis of IL-6, IL-lα, IL-lβ and macrophage inflammatory protein (MIP)-1α compared to TNF +/+ LT +/+ mice. No differences in the response of mice of both groups to infection were observed if the animals were induced to become neutropenic.

With disseminated candidiasis in IL-6 - deficient mice (IL-6 -/-), more massive contamination of organs with the fungus and increased mortality were noted compared to the control group of IL-6 +/+ animals . IL-6 -/- mice, despite an increase in plasma concentrations of TNF-α, IL-1α and IL-1β, are not capable of an effective neutrophil response to C. albicans In cyclophosphamide-induced neutropenia, IL-6 -/- and IL-6 +/+ animals are equally susceptible to candidiasis.

In resistance to Candida it is difficult to overestimate the protective effect of granulocyte colony-stimulating factor (G-CSF). In an artificially created line of G-CSF and G-CSF receptor-deficient mice, neutropenia and defects in myelo- and granulocytopoiesis occur. In experimental candidiasis, administration of exogenous G-CSF to mice increases the number of PMNs in circulation, reduces the contamination of the kidneys with the fungus, and increases the survival of animals.

It is known that INF-y is the most powerful stimulator of the effector functions of macrophages (microbicidal activity, cytokine production), increases the expression of MHC1 and MHCII, as well as adhesion molecules on endothelial cells, increasing endothelial permeability. It limits the proliferation of the fungus in the early stages of candidiasis and stimulates the immune response. In mice with a genetically determined absence of the IFN-γ receptor (IFN-γ R -/- mice), the candidal activity of macrophages is significantly reduced. In experimental candidiasis, the contamination of the kidneys, liver, and spleen with the pathogen in such mice is significantly higher, and antibody titers during the secondary immune response are lower than in the control IFN-γ R +/+ group of animals. In patients with CCCS, there is often a decrease in the synthesis of IFN-γ.

The significant significance of IL-8 in the regulation of PMN function is evidenced by the high density of receptors on cells (60,000 per neutrophil) for this cytokine. Germ-free BALB/c mice genetically determined to be IL-8 receptor deficient (IL-8 R -/-) are highly susceptible to local and disseminated candidiasis. In IL-8 R^" animals, a slowdown in the migration of PMNs into infected tissues, a decrease in the oxidative activity of phagocytes was noted, and PMNs interfered with hypha formation to a lesser extent than the cells of IL-8 R +/+ animals.

In experimental candidiasis in neutropenic mice, recombinant IL-12 significantly increases the effectiveness of antifungal therapy. The importance of IL-12 in candidiasis is determined by its participation in the differentiation of Th 0 CD4 + towards Th type 1, its ability to stimulate the functional maturation of cytotoxic CD8 + lymphocytes and the production of INF-y. The human immunodeficiency virus inhibits the synthesis of IL-12, which apparently makes a significant contribution to the development of insufficiency of cellular immunity in AIDS.

A study of anti-inflammatory cytokines has shown their ambiguous effect on the course of candidiasis. In IL-10, mice are “knocked out” one day after intravenous infection with C. albicans Kidney damage by the fungus did not occur, unlike the control immunocompetent group of animals. Moreover, the increased resistance of experimental mice correlated with a more pronounced killing activity of PMNs against C. albicans On the contrary, mice lacking the gene encoding the synthesis of IL-4 were more susceptible to systemic candidiasis than controls, while when animals were orally infected, there were no significant differences in susceptibility to candidiasis between IL-10-, IL-4-knockout mice and wild immunocompetent strain. In mice with disseminated candidiasis reproduced against a background of neutropenia, administration of soluble IL-4 receptors significantly increased the therapeutic effect of antifungal drugs. Apparently, neutralization of IL-4 by the corresponding receptors reduced the inhibitory effect of this cytokine on the production of IL-1 and IFN-y and thus stimulated cellular immunity.

Candidiasis is often accompanied by stimulation of the production of the anti-inflammatory cytokine transforming growth factor TGF-β. The effect of the latter is manifested, in particular, by suppression of the activity of monocytes, inhibition of proliferation and functional activity of T- and B-lymphocytes, as well as the synthesis of immunoglobulins of the class G. Incubation of human peripheral blood monocytes with WITH. albicans stimulates the excretion of TGF-P. In patients with candidiasis, a significant amount of TGF-β was found in biopsies of inflammatory granuloma. It is believed that local synthesis of this cytokine by infected mononuclear cells is a component of the response to infection caused by C. albicans, and may be one of the causes of immunosuppression.

Despite the progress achieved in the study of cytokines, their use in clinical practice is not always effective, which is apparently due to a disruption of the cell receptor apparatus.

For immunodeficiency conditions caused by disorders of humoral immunity, candidiasis is not a characteristic disease. At the same time, numerous experimental and clinical data indicate the protective significance of antibodies (AT) during candidiasis. Antiserum obtained by immunizing mice with blastospores of C. albicans, as well as IgM and IgG3 monoclonal antibodies to beta-1,2-mannotriaose (B6.1 epitope, evenly distributed over the entire surface of the cell wall of C. albicans) reduce the mortality of animals from disseminated candidiasis and reduce the contamination of the vagina during vaginal candidiasis. In intensive care unit patients, a direct correlation has been shown between low titers of AT to Candida in blood serum and mortality from candidiasis, Local application of bovine immunoglobulins obtained by immunizing animals with several species of fungi of the genus Candida, has a protective effect against oral candidiasis.

Humoral immune response to Candida spp. has a number of features that distinguish it from the response to many other antigens. Fungi have relatively low immunogenic properties; the magnitude of the humoral immune response to them is not as significant as to viruses and many bacteria. Cell wall Candida spp. has a strong structure, and activation of the complement system does not lead to lysis of fungal cells. In conditions of an immunized organism, in the absence of direct contact with S. albicans with blood cells, AT does not inhibit the proliferation of the fungus. On the contrary, the fungus continues to actively vegetate, and glycogen reserves are found in the filaments of pseudomycelium. The protective effect of AT against candidiasis is carried out mainly indirectly due to their opsonizing properties, which limits the proliferation of the fungus in the body. Widespread occurrence of opportunistic pathogens Candida spp. in the environment, the presence of common intrageneric antigens, as well as antigens that cross-react with Saccharomyces cerevisiae(true yeast, widely used in the food industry), promotes the accumulation of natural AT that reacts with Candida spp. Such ATs are present in the blood serum of almost all people, and their peculiarity is their exceptionally high neutralizing activity against Candida. This property apparently allows natural ATs to effectively carry out their functions already at the stage of the initial meeting of the organism with the pathogen.

The level of AT synthesis during candidiasis is largely determined by the depth and extent of the pathological process. In patients with superficial forms of infection (candidal dermatitis, intertriginous candidiasis, paronychia, folliculitis) stimulation of the humoral immune response to Candida, usually not observed. In other forms of infection, AT titers are often elevated.

On the severity of the humoral immune response to antigens Candida The genetic determination of the immune response has a significant impact. A model of oral candidiasis has shown, for example, that after an infection, IgG-AT titers to C. albicans in blood serum and IgA-AT in saliva in BALB/c mice is higher than in DBA/2 (H-2 (d)). A relationship has been established between the severity of the humoral immune response to C. albicans with the HLA system. The highest type of response was observed in people who have the HLA-B8 antigen, which is associated with reduced suppressor activity of CD8 + T cells in these individuals.

Many authors note that in individuals who are practically healthy and those with candidiasis of the mucous membranes, antibodies are most often synthesized to a wider range of fungal antigens than in deep forms of the disease. Thus, in healthy people, IgG-AT is detected against the α- and β-1,2-oligomannosidic epitopes of mannan C. albicans(missing from S. crevisiae), while in visceral candidiasis ATs are usually synthesized against the α-, but not the β-1,2-oligomannosidic epitope. Our team of authors has shown that in case of oral candidiasis and vulvovaginal candidiasis, the most significant increase in AT titers occurs to the total cell wall antigen C. albicans, while in visceral forms of infection and CCKS, IgG-AT is synthesized to a greater extent to the glycoprotein fraction of the cell wall with a molecular weight of 62-85 kDa. According to other data, in patients with mucosal candidiasis there is an increase in antibody titers to antigens with a molecular weight of 47 kDa and 29 kDa, while in invasive forms of infection - mainly (in 90-92% of cases) to the 47 kDa antigen. Absence of antibodies to certain epitopes Candida, apparently not associated with the formation of immune complexes. Among patients with various forms of candidiasis, specific circulating immune complexes containing cell wall antigen C. albicans, are determined in no more than 30% of patients. Modification of the humoral immune response to antigens Candida in the process of development of deep forms of candidiasis, it is more likely due to the variability of the fungus or the development of tolerance to a number of its antigenic determinants.

Cell wall polysaccharides of yeast and yeast-like fungi have a nonspecific stimulating effect on the humoral immune response. If in practically healthy individuals the amount of AT of high avidity to Candida is about 87%, then in patients with chronic genital candidiasis and CCCS - 79% and 67%, respectively.

WITH. albicans has cross-reacting antigens with many human tissues: skin, mucous membranes of the gastrointestinal tract, a number of endocrine organs, etc. Animal experiments have shown that, under certain conditions, candidiasis can cause a breakdown in tolerance to self-antigens and the development of immunopathological reactions.

In CCCS, an increase in the level of autoantibodies to antigens of various tissues (adrenal glands, thyroid and parathyroid glands, organs of the gastrointestinal tract), an increase in the immunoregulatory index “CD4V CD8^” due to a decrease in the number of CD8 + T cells (a subpopulation of cells that includes T cells with suppressor activity) are often detected. ), and in 64.5% - 88.9% of cases - the development of immediate hypersensitivity to the allergen Candida, promoting increased vascular permeability and extravasation of proteins from the circulation into the tissue. Violation of the structure of the vascular wall can also be caused by the local action of proteinases C. albicans. The complex of these factors creates the preconditions for the development of immunopathological reactions with damage to the relevant organs. Thus, it has been noted that in patients with CCCS, endocrinopathy often develops several years later than candidiasis.

In 5 out of 10 patients with CCCS complicated by candidal esophagitis, we found in the circulation increased titers of AT to the antigen of the esophageal mucosa, and in the lesions of the esophagus - immune complexes containing the S3 component of complement and AT to the antigen of this organ. In the blood serum of such patients, a decrease in the direct and alternative pathways of complement activation, the concentration of C3 and C4 components of complement, as well as a significant increase in the content of IgG3 and lgM immunoglobulins, which, being part of immune complexes, activate complement to the greatest extent, were revealed. Thus, cellular immunity plays a special role in protecting the body from candidiasis, taking an active part in limiting the spread and destruction of cells Candida spp. Dysfunction of the immune system can contribute to the development of severe forms of candidiasis, and under certain circumstances, the occurrence of immunopathological reactions that aggravate the course of the underlying disease.
