Action and application.

Prozerin blocks cholinesterase, prevents the hydrolytic cleavage of acetylcholine, which, accumulating, enhances the contractile activity of the uterus, stomach, intestines, constricts the pupils, and lowers intraocular pressure. It restores the course of the acetylcholine cycle in motor nerve fibers and endings, stimulates the work of neurons, increases the tone and activity of nerves.

It acts similarly to physostigmine, weaker on carnivores and omnivores, and stronger on horses and cattle.

Stimulates the secretion of the digestive glands, increases tone and increases the contraction of the smooth muscles of the digestive tract, uterus, bladder and skeletal muscles.

Changes the resting stage of the estrous cycle. It is used for barrenness of cows, injected subcutaneously at 0.01 g in the form of a 0.5% solution three times.

A 0.05% solution of prozerin is used in animals with weakness of labor, subinvolution of the uterus, retained placenta, endometritis, ovarian hypofunction, for the prevention and treatment of postoperative atony of the gastrointestinal tract of the bladder, as a ruminator. It is used for paresis and paralysis of the motor nerves, motor disorders after brain injuries, various diseases of the nerve trunks and fibers with a decrease in their functions, myasthenia gravis, optic nerve atrophy, neuritis. It is an antagonist of curare-like drugs.

With atony of the proventriculus in ruminants, atony of the bladder is also used subcutaneously 2 times a day.

With myasthenia gravis, the drug is prescribed orally or under the skin 2 times a day for 20-30 days.

The drug is administered to animals subcutaneously according to the following scheme: to cows to stimulate labor activity 3-4 ml 3 times with an interval of 1 hour, with retention of the placenta 3-4 ml twice with an interval of 12 hours, to prevent and treat subinvolution of the uterus, acute and chronic endometritis 3-4 ml with an interval of 48 hours in combination with other methods of treatment, with the ovarian cyst - after 3 days. , with hypofunction of the ovaries 2-3 ml once in combination with FFA, which is administered subcutaneously 24 hours after the injection of prozerin at a dose of 500 ME per 100 kg of body weight, pigs with the same indications 1-2 ml.

Prozerin is prescribed for overdose of antidepolarizing muscle relaxants as an antidote in the same doses.

In eye practice, a 0.5% solution is used, several drops 2-3 times a day.

In case of overdose, poor tolerance and ingestion of prozerin solution into muscle tissue possible side effects due to shortness of breath, enteritis, salivation, anxiety. In such cases, the administration of the drug is stopped. As an antidotan, atropine is prescribed subcutaneously at the rate of 0.05 mg/kg of body weight.

Side effect. From the digestive system: nausea, vomiting, diarrhea, hypersalivation, flatulence, spastic contraction and increased peristalsis.

From the side of the central nervous system and peripheral nervous system: headache, dizziness, weakness, loss of consciousness, drowsiness, miosis, visual disturbances, twitching of skeletal muscles (including the muscles of the tongue), convulsions.

From the side of the cardiovascular system: arrhythmias, brady- or tachycardia, AV blockade, junctional rhythm, non-specific ECG changes, lowering blood pressure.

From the respiratory system: shortness of breath, respiratory depression, increased secretion of bronchial glands, increased tone of the bronchi.

Allergic reactions: possible skin rash, itching, anaphylactic reactions.

Others: arthralgia, increased urination, increased sweating.

Overdose.Symptoms:

associated with overexcitation of cholinergic receptors (cholinergic crisis): bradycardia, hypersalivation, miosis, bronchospasm, nausea, increased peristalsis, diarrhea, frequent urination, twitching of the muscles of the tongue and skeletal muscles, the gradual development of general weakness, a decrease in blood pressure. Treatment: reduce the dose or stop treatment, if necessary, introduce atropine (1 ml of a 0.1% solution), metacin and other anticholinergic drugs.

Contraindications. Epilepsy, hyperkinesis, arrhythmias, bradycardia, thyrotoxicosis, peptic ulcer of the stomach and duodenum, peritonitis, mechanical obstruction of the gastrointestinal tract or urinary tract, acute infectious diseases, intoxication of the weakened, hypersensitivity to neostigmine methyl sulfate.

Prescribed with caution against the background of anticholinergics, with myasthenia gravis against the background of neomycin, streptomycin, kanamycin and other antibiotics that have an antidepolarizing effect, local and some general anesthetics, antiarrhythmics and a number of other drugs that disrupt cholinergic transmission.

Use during pregnancy and lactation.

Neostigmine methyl sulfate crosses the placental barrier and is excreted in very small amounts in breast milk.

Prozerin during pregnancy and lactation is used only according to strict indications.

When myasthenia gravis is prescribed in combination with aldosterone antagonists, corticosteroids and anabolic hormones. Atropine, metacin, etc. M-anticholinergics weaken M-cholinomimetic effects (pupil constriction, bradycardia, increased gastrointestinal motility, hypersalivation, etc.). It lengthens and enhances (when administered parenterally) the effect of depolarizing muscle relaxants (ditilin, etc.), weakens or eliminates antidepolarizing ones.
