To issue a referral coupon (quota) for receiving high-tech medical care at FBSU National Medical Research Center for Surgery named after A.V. Vishnevsky, the patient must contact the regional health authority with the following documents:

  • an extract from the patient's medical documentation must contain information about his health status, the examination and treatment performed, recommendations on the need to provide an IUD, the results of clinical and diagnostic studies on the profile of the disease.

If the region of the Russian Federation does not have quotas for referring a patient for treatment to the National Medical Research Center for Surgery named after A.V. Vishnevsky, a citizen has the right to apply to the Ministry of Health of Russia (Moscow, Rakhmanovsky per., 3, Department of high-tech medical care).

Residents of Moscow should apply for a quota to the City Department of Health (Moscow, 2nd Schemilovsky per., 4 A, building 4). Opening hours: Monday-Thursday from 9.00 to 17.30, Friday from 9.00 to 16.30 (lunch break from 13.30 to 14.30).

You must have the following documents with you:

  • passport of the person for whom the referral coupon is issued
  • photocopy of passport (2,3 pages + registration)
  • a photocopy of the compulsory health insurance policy
  • photocopy of state pension insurance certificate
  • a photocopy of the disability certificate (if any).

All citizens of the Russian Federation, without exception, have the right to receive free high-tech medical care (HMP). The main condition for obtaining a HMP is the appropriate medical indications (clause 5 of article 10, part 3 of article 34 of the Law of 21.11.2011 N 323-FZ).

VMP is part of specialized medical care and includes the use of new complex and (or) unique treatment methods, as well as resource-intensive treatment methods with scientifically proven efficacy, including cellular technologies, robotic technology, information technology and genetic engineering methods developed on the basis of achievements of medical science and related branches of science and technology ( p. 2 Order approved. By order of the Ministry of Health of Russia dated December 29, 2014 N 930н).

VMP turns out to be in accordance with the List of VMP types included in the basic CHI program, and the List of VMP types not included in the basic CHI program. Regardless of the type, the HMP is free of charge, since it is included in the Program of state guarantees of free provision of medical care to citizens and is financed by the FFOMS (clause 1 of part 5 of article 80 of Law N 323-FZ; part 2 - 3 of article 35, article 50.1 of the Law of November 29, 2010 N 326-FZ; part 5 of article 5 of the Law of December 19, 2016 N 418-FZ; paragraph 3 of section II of the Program, approved by the Decree of the Government of the Russian Federation of December 19, 2016 N 1403).

Step 1. See your doctor

First of all, you should contact your doctor in order to obtain a referral for hospitalization, draw up the necessary documents and send them for consideration to a competent organization. The attending physician of the medical organization in which the patient undergoes diagnostics and treatment in "normal" conditions determines the presence of medical indications for the provision of an upper urinary tract (clause 11 of the Procedure).

The indications for the VMP are diseases and (or) conditions requiring the use of the VMP in accordance with the List of types of the VMP (clause 12 of the Procedure).

The presence of medical indications is confirmed by the decision of the medical commission of the medical organization, which is drawn up in a protocol and entered into the patient's medical documentation (clause 11 of the Procedure).

If there are medical indications, the attending physician draws up a referral for hospitalization (clause 13 of the Procedure).

Reference. Requirements for a referral for hospitalization and its annexes

1. The referral must be filled out on the letterhead of the sending medical organization legibly by hand or in printed form, certified by the personal signatures of the attending physician and the head of the medical organization, as well as by the seals of the attending physician and medical organization ( p. 13 of the Procedure).

2. The direction should contain the following information ( p. 13.1 - 13.7 Order):

- FULL NAME. patient, date of birth, registration address;

- the number of the compulsory medical insurance policy and the name of the medical insurance organization;

- insurance certificate of compulsory pension insurance;

- the code of the diagnosis of the underlying disease in accordance with the international classification of diseases;

- profile and name of the type of VMP;

- the name of the medical organization to which the patient is sent;

- FULL NAME. and the position of the attending physician, if available - his phone number and email address.

3. You will need to attach ( p. 14.1 - 14.3 Order):

- an extract from medical records indicating the diagnosis of the disease, the disease code according to the international classification of diseases, information about the state of health, the results of special medical research. The extract must be certified by the personal signatures of the attending physician and the head of the medical organization;

- a copy of the patient's identity document or a copy of the birth certificate (for children under 14);

- a copy of the compulsory medical insurance policy (if any);

- a copy of the insurance certificate of compulsory pension insurance (if any);

- consent to the processing of personal data.

The head of the referring medical organization or another employee of the medical organization authorized by the head, within three working days, sends a referral for hospitalization, including through a specialized information system, postal and (or) electronic communication (clause 15 of the Procedure):

  • to the receiving medical organization, if the VMP is included in the basic compulsory medical insurance program (clause 15.1 of the Procedure);
  • to the executive authority of the constituent entity of the Russian Federation in the field of health care (HPH), if the HMP is not included in the basic compulsory medical insurance program (clause 15.2 of the Procedure).

Note. The patient or his legal representative has the right to submit the completed package of documents independently ( p. 16 of the Procedure).

Step 2. Wait for the VMP ticket to be issued

A coupon for the VMP is issued using a specialized information system.

If the patient is referred to the provision of a VMP included in the basic compulsory medical insurance program, the receiving medical organization (clause 17 of the Procedure) provides a coupon for the provision of a VMP with the attachment of the set of documents specified in step 1.

If the patient is directed to the provision of high-tech medical care, which is not included in the basic compulsory medical insurance program, the issuance of a coupon for the provision of high-medical care with the attachment of the set of documents specified in step 1, and the conclusion of the commission of the executive authority of the constituent entity of the Russian Federation in the field of healthcare on the selection of patients for the provision of high medical care (OUH commission) provides OUZ (clause 18 of the Procedure).

The OUH Commission makes a decision on the presence (absence) of indications for referring the patient to the receiving medical organization within 10 working days from the date of receipt of the full package of documents. The decision of the OUZ commission is formalized in a protocol (clause 18.1 of the Procedure).

The protocol of the OUZ commission must contain a conclusion on the presence (absence) of indications for referral to the VMP or on the need for additional examination (clause 18.2.5 of the Procedure).

Note. An extract from the protocol of the decision of the OUH commission is sent to the sending medical organization, including via postal and (or) electronic communication, and is also handed over to the patient (his legal representative) upon written application or sent to the patient (his legal representative) by mail and ( or) electronic communication ( p. 18.4 of the Procedure).

Step 3. Wait for the decision of the commission of the medical organization providing VMP and hospitalization

The basis for hospitalization of a patient in medical organizations providing VMP is the decision of the medical commission of the medical organization to which the patient is sent, on the selection of patients for the provision of VMP (commission of the medical organization providing VMP) (clause 19 of the Procedure).

The commission of a medical organization providing a high-tech medical care makes a decision on the presence (absence) of medical indications or the presence of medical contraindications for hospitalization of a patient within seven working days from the date of issuing a coupon for the provision of high-tech medical care (except for cases of emergency, including specialized emergency, medical care) ) (clause 19.2 of the Procedure).

The decision of the commission of a medical organization providing high-tech medical care is drawn up in a protocol containing a conclusion on the presence of medical indications and the planned date of hospitalization of the patient, on the absence of medical indications for hospitalization, on the need for additional examination, on the presence of medical indications for referring the patient to a medical organization for the provision of specialized medical assistance, on the presence of medical contraindications for hospitalization of the patient in a medical organization providing high-tech medical care (clause 5, clause 19.3 of the Procedure).

An extract from the protocol of the commission of the medical organization providing high-tech medical care, within five working days (but not later than the planned hospitalization period) is sent via a specialized information system, postal and (or) electronic communication to the sending medical organization and (or) the health care institution that issued a coupon for provision of VMP, as well as handed over to the patient (his legal representative) upon written application or sent to the patient (his legal representative) by mail and (or) electronic communication (clause 20 of the Procedure).

Note. If there are medical contraindications for hospitalization of a patient to a medical organization providing a high-tech medical device, the refusal to hospitalize is marked with a corresponding entry in the coupon for the provision of high-pressure medical devices ( p. 20 of the Procedure).

Step 4. Upon completion of the provision of the VMP, you will receive recommendations

Based on the results of the provision of VMP, medical organizations give recommendations for further observation and (or) treatment and medical rehabilitation with the registration of appropriate records in the patient's medical documentation (clause 21 of the Procedure).

Note. In case of dissatisfaction with the quality of the provision of high medical care, you have the right to contact local health authorities or territorial bodies of Roszdravnadzor ( p. 4 Resolutions of the Government of the Russian Federation of 06.04.2004 N 155; h. 2 tbsp. 9 of Law N 323-FZ).

It should be borne in mind that for some categories of citizens there is a special order of referral for the provision of HMP (clause 22 - Order).

Treatment of some diseases is so complex and costly that citizens are not able to pay and organize it on their own. But every citizen of the Russian Federation has guarantees from the state, written down in the basic law. They are provided with quotas for specialized medical services.

Diseases subject to quotas


The state does not give money to rid a citizen of any ailment. Good reasons are needed to obtain a quota.

The Ministry of Health issues a document containing a list of diseases that are subject to treatment at public expense. The list is extensive, it contains up to 140 ailments.

Here is some of them:

  1. Diseases of the heart, to get rid of which surgery is indicated (including repeated).
  2. Internal organ transplant.
  3. Joint prosthetics, if endoprosthetics is necessary.
  4. Neurosurgical intervention.
  5. In Vitro Fertilization (IVF).
  6. Treatment of severe hereditary diseases, including leukemia.
  7. Surgical intervention requiring specialized equipment, that is, high-tech medical care (HMP):
    • before our eyes;
    • on the spine and so on.
The Ministry of Health of the Russian Federation determines the number of quotas for each institution that has an appropriate license. This means that the respective clinic can only accept a certain number of patients for treatment at the expense of the budget.

The procedure for obtaining a privileged place in the clinic

The path to a medical facility that can cure is not easy. The patient will have to wait for a positive decision from three commissions. This procedure for obtaining a quota was established by the Ministry of Health of the Russian Federation.

There is a workaround. We will describe it a little later. Any application for a quota should be started with the attending physician.

To receive preferential treatment, you need to confirm the diagnosis. This may require paid tests and examinations. The patient will have to make their own savings.

The first commission - at the place of observation of the patient

The sequence for initiating the receipt of a quota is as follows:

  1. Contact the treating Aesculapius and describe the intention.
  2. Get a referral from him if you need to undergo additional examination. Failure to do so will result in non-receipt of the quota.
  3. The doctor draws up a certificate, which indicates the data:
    • about the diagnosis;
    • about treatment;
    • on diagnostic measures;
    • about the general condition of the patient.
  4. The certificate is considered by the commission that deals with the solution of quota issues, created in this medical institution.
  5. This body is given three days to make a decision.
The treating doctor is responsible for the “candidate” for the quota. He cannot recommend to the commission a citizen who can do without a VMP.

Decision of the first commission

If the patient needs specialized services, the hospital commission decides to send the documents to the next body - the regional health department. At this stage, a package of documents is formed, which includes:

  1. An extract from the minutes of the meeting with a justification for the positive decision;
  2. A photocopy of the passport (or birth certificate, if we are talking about a child under 14 years old);
  3. The statement in which you need to reflect:
    • registration address;
    • passport data;
    • citizenship;
    • contact information;
  4. A copy of the OM S policy;
  5. Pension insurance policy;
  6. Insurance account data (in some cases);
  7. Survey and analysis data (originals);
  8. Extract from the medical card with a detailed diagnosis (prepared by the doctor).
It is necessary to give the consent of the medical organization to the processing of personal data. For this, another statement is being written.

Second stage of decision making


The regional level commission includes five specialists. Its activities are directed by the head of the relevant department. This body is given ten days to make a decision.

If a positive decision is made, this commission:

  • determines the medical institution in which the treatment will be carried out;
  • sends a package of documents there;
  • informs the applicant.
It is customary to choose a clinic located near the patient's place of residence. However, not all hospitals are licensed to perform specialized operations. Consequently, a citizen may well be given a referral to another region or to a capital institution.

The work of this body is recorded. The following data is reflected in the paper:

  • the basis for the creation of a commission of a constituent entity of the Russian Federation;
  • the specific composition of the sitting persons;
  • information about the patient whose application was reviewed;
  • conclusion, which deciphered:
    • complete data on indications for granting quotas;
    • diagnosis, including its code;
    • grounds for referral to the clinic;
    • the need for additional examination;
    • grounds for refusal upon receipt of a VMP.

The following are sent to the medical institution, where the patient will be provided with a VMP:

  • coupon for the provision of VMP;
  • copy of the protocol;
  • medical information about human health.

The third stage is the final

There is also a quota commission in the medical institution chosen for the treatment. After receiving the documents, she holds her own meeting, in which at least three people must take part.

This body:

  1. Examines the information provided for the possibility of carrying out the treatment necessary for the patient
  2. Decides on its provision.
  3. Determines specific deadlines.
  4. He is given ten days for this work.
The coupon, if used, is kept at this clinic. It is the basis for budgetary financing of treatment.

Thus, the decision to include a person in the quota program takes at least 23 days (you should also take into account the time for sending the documentation).

Features of quota services


Only those medical services that cannot be obtained at the local hospital are provided with public funds.

Their types are as follows:

  • surgical intervention;
  • treatment.
Each type of assistance requires specialized equipment, appropriate training of specialists. That is, common diseases are not subject to quotas.

Operation

This type of support is provided to people whose diagnosis complies with the list of the Ministry of Health. They are sent to a clinic capable of performing the necessary manipulation. All treatment is provided to them free of charge.

Some citizens are also paid for travel to the place of assistance.

VMP

This type of service involves the use of high technology to get rid of the disease. This is an expensive procedure. All necessary expenses are covered by the budget.

However, there is a compelling medical reason for the provision of a VMP.

Treatment

This type of state support implies the purchase of expensive drugs, which the patient is unable to pay for. Its procedure is determined by Federal Law No. 323 (Article 34). The Government of the Russian Federation specifies the implementation into practice of the provisions of this normative act by its decrees.

ECO

Women who have been diagnosed with infertility are sent for such an operation. In vitro fertilization is a costly and time-consuming procedure.

Many women are not able to feel the joy of motherhood without such an operation. However, IVF referrals are issued only to patients who have gone through a difficult preliminary period of examinations and treatment.

Not all types of assistance in restoring health and preserving the life of a citizen of the Russian Federation have been described. There are many ailments, almost all of them fall under one of the described areas of medical technology. But there are also exceptions.

How to shorten the time to get support


People often do not have the opportunity to wait. Help is needed urgently.

Accelerating the decision-making process by three commissions is not easy.

In the first case, you can put "pressure" on the people responsible for the allocation of quotas:

  • call them to find out about the progress in resolving the issue;
  • go to appointments with managers;
  • writing letters and so on.
The effectiveness of this method is questionable. Only experienced specialists take part in the work of the commissions. These people themselves understand that the delay is unacceptable.

The second option is to go directly to the clinic providing the required services. This requires:

  • collect a package of documents (described above);
  • bring to the hospital and write a statement on the spot.

Documents from the local hospital where the patient was initially diagnosed must be certified by:

  • attending doctor;
  • chief physician;
  • with the seal of the organization.

Unfortunately, the quota clinic will not be able to provide assistance without observing the formalities. This medical institution has yet to account for the use of budget funds.

1. When providing VMP at the expense of the compulsory medical insurance funds, citizens' documents are sent directly to the medical organization providing VMP in one of the following ways:

2. When providing HMP at the expense of the budget, citizens' documents are sent to the Commission of the Health Committee of St. Petersburg for the selection of citizens for the provision of HMC in one of the following ways:

a patient in person at the City Organizational and Methodological Department for VMP (OMO VMP) (St. Petersburg, Shkapina St., 30, room 216; metro station Baltiyskaya). Reception of documents is carried out on Monday and Friday from 10.00 to 14.00, on Wednesday from 13.00 to 17.00;

The procedure for registration of documents at the VMP

1. Registration by the attending physician extracts from medical records.

2. Registration by the attending physician referrals for hospitalization for the provision of high-tech medical care.

3. Provided by the patient or his legal representative consent to process the personal data of the patient or his legal representative.

4. Submission by the sending medical organization of a set of documents within three working days, including through a specialized information system, postal or electronic communications:

- in the case of providing a high-tech medical facility included in the basic compulsory medical insurance program, to a medical organization included in the register of medical organizations operating in the field of compulsory health insurance;

- in the case of the provision of a high-tech medical facility that is not included in the basic compulsory medical insurance program (at the expense of the federal and city budgets), to the VMP department of the St. Petersburg MIAC by the address:

4.1. The patient or his legal representative has the right to independently submit a completed set of documents to the specified institutions within three working days from the date of referral to hospitalization.

4.2. A package of documents for the provision of high medical care, not included in the basic compulsory medical insurance program, the patient or his legal representative can submit to the high medical care department of St. Petersburg GUBZ MIAC in one of the following ways:
- personally at the address: st. Shkapina, 30, lit. А, office 216 (floor 2);

The packages of documents in the HMP department of SPB MIAC are checked, submitted to the meeting of the Commission of the Health Committee on the selection and referral of citizens for the provision of HMP, followed by the issuance of a coupon for the provision of HMP in the specialized information system of the Ministry of Health of the Russian Federation.

5. Consideration of the received documents by the Committee on Healthcare - within 10 working days from the date of their receipt.

6. Registration of the "Coupon for the provision of VMP" in case of a positive decision.

8. Commission medical organization providing a VMP, makes a decision on the presence (absence) of medical indications for hospitalization of the patient, taking into account the types of VMP provided by the medical organization, included in the list of types of VMP, within a period not exceeding 7 working days from the date of issuing a coupon for the provision of VMP (except for cases provision of ambulance, including specialized ambulance, medical care).

9. The patient can view information about his referral coupon for the provision of VMP by the number located at the top of the first page of the referral coupon on the official website of the Ministry of Health of Russia, at talon.rosminzdrav.ru. At the end of the calendar year, in case of failure to provide a VMP in the current year, the coupon is blocked by the Ministry of Health, since a coupon with a new number is created instead of it in the order of the available queue.
