Every year on the pages of the newspaper we talk about positive changes in the Sheksninskaya Central District Hospital - repairs carried out, updating of the material and technical base, events aimed at improving quality and accessibility medical care for the population of the Sheksninsky district. Work is constantly being carried out to control accounts payable; from May 2016 to the current date, there has been no delay in payments to counterparties or other overdue debt.

Within a year, we plan to hire eight experienced doctors and young specialists from other regions, and five mid-level staff. The agreement has already been received.
- Alexander Vladimirovich, you have specialists constantly moving from office to office, from floor to floor. People who have been accustomed to one layout system for years get lost, although they later admit that things have not gotten worse, but on the contrary, there is more space, light, and convenience. Explain why these constant housewarming parties?

- We bring the premises of outpatient care in accordance with regulatory documentation. Relocations occur, as you probably noticed, only to renovated offices. I hope that the patients noted the spaciousness at the ENT and surgeons’ office; even the queues seem to have become shorter - we have removed the attached oncology specialist’s office, giving him a normal room. The buffet moved from the first floor to the second, we entered into an agreement with one of the cafes in Sheksna, and now here you can have a full lunch, drink tea - everything is civilized, pleasant, tasty. The vacant premises of the former canteen are now given over to the emergency department; we installed double-glazed windows there and carried out renovations. Partitions former premises The emergency departments on the second floor have been removed, now there are offices here and all the local therapists have moved into them. In the future, the treatment and vaccination rooms will be moved here. Gradually the system will be built, and our patients will get used to it. Moreover, everything is done for their convenience and comfort.

The first laparoscopic surgery was performed in Sheksna

In the autumn interview chief physician Sheksninskaya Central District Hospital A.V. On December 20, endoscopic equipment for laparoscopic operations was received. It was tested, and on December 22 the first patient was operated on. This became the reason to get to know the head of the surgical department better and ask him about the prospects for Sheksna surgery.
­ - Boris Anatolyevich, who became the first patient of Sheksna’s unique operation?
“We just had a patient in the surgical department who was admitted with an exacerbation of cholelithiasis. The issue of performing an open or, as it is also called, strip operation on him was being decided, since an acute attack could result in serious consequences. He was lucky, and we also had the luck, one might say, to test the equipment in action right away. Now, instead of a huge scar and a painful long postoperative recovery, he has only four small sutures measuring 1-1.5 cm. He underwent a cholecystectomy, that is, removal of the gallbladder, and with it two stones of very impressive size. Postoperative period went smoothly. The next day he was able to get up and walk. ­ - For you personally, was this the first such operation or have you had to do them before?
- Before moving to Sheksna, I worked for 23 years at the Ustyuzhensk regional hospital and had about 200 similar operations on my account. This method has been introduced into surgical practice there since 2009. Endoscopic surgery versus traditional method has many advantages - such operations are less traumatic, the body recovers faster after them, and there are no large scars left on the body. And using this method, it is possible to perform operations not only to remove the gallbladder, but also, for example, appendicitis, and gynecological surgical interventions.
- Boris Anatolyevich, despite your extensive medical experience, you are new to our hospital, but you have already earned the authority and respect of patients. In your opinion, will Sheksna surgery be able to reach the heights for which the department has always been famous?
- The team of surgeons is efficient, the specialists are strong. It is important that we are fully staffed with anesthesiologists. Modern appears diagnostic equipment. The head physician is very interested in equipping the surgery with new equipment for carrying out operations on modern techniques, on the introduction of advanced technologies. The equipment of the clinical laboratory greatly helps us in successful treatment sick. With the introduction of endoscopic surgery into practice, the list of operations that we can perform will expand. So, one of the closest options is unipolar hip replacement. We have already received metal structures and unipolar prostheses. We are waiting for the equipment, the contract for its supply has been concluded. - I was born in Veliky Ustyug, a fellow countryman of our chief physician - Alexander Vladimirovich Lugovin. Since the seventh grade I already wanted to become a doctor. My mother is a teacher of Russian language and literature, my father is a water transport engineer, but my uncle is a general practitioner, so there were doctors in our family. After finishing school he entered Arkhangelsk medical school. A year before its end, I wrote letters to hospitals in the region, told about myself, as they say today, and sent a resume. I wanted to decide on a place of work in advance. A very warm letter came from the chief physician of the Ustyuzhensk regional hospital. I went to Ustyuzhna, I liked the city, and the working conditions too. After my internship, I spent 23 years working on health issues for the people of Ustyug. My son Mikhail was born and graduated from school in Ustyuzhna. Now he is a second-year student at the Arkhangelsk Medical Academy. I hope he will follow in my footsteps and become a surgeon too. As for hobbies, the main thing is my work. Apart from that I like to read good books, play chess and dream of mastering the guitar.

What was 2016 like for the Sheksninskaya Central District Hospital?

During the year, the Sheksninskaya Central District Hospital took measures aimed at improving the quality and accessibility of medical care for the population of the region. Thus, the second floor of the ward building of the hospital was renovated, where the provision of outpatient care for the population was organized, measures were carried out according to the instructions of the supervisory authorities in previous years (fire prevention treatment of the premises of the clinic of the central district hospital and first aid stations, recharging and replenishment of all available fire extinguishing equipment, anti-terrorism security measures) . The roofs of the catering unit, ward and administrative buildings of the hospital were repaired. A significant achievement is the purchase of medical equipment, specialized equipment and office equipment for a total amount of more than three million rubles (endoscopic equipment, two dental units, a plasma defroster, an audiometer, defebrillators, a urine analyzer and other equipment), medical furniture was purchased. Under donation agreements, office furniture, office equipment, construction materials were received and other assistance was provided for the hospital.

It arrived in our regiment
Young medical specialists, doctors with experience and intermediate medical staff. In addition, they are aimed at advanced training and training, including specialized ones medical specialties, more than 60 hospital employees. All the measures taken will improve the quality of medical care provided to the population of the Sheksninsky district, and in the future - increase the level of prevention, diagnosis and treatment, and reduce the incidence rate.
According to the federal program “Zemsky Doctor” and in order to implement the regional law of 06.05. 2013 No. 3533OZ “On social support measures aimed at staffing the regional healthcare system”, eight medical specialists who came to work at the Sheksninskaya Central District Hospital in 2016 were attracted and received one-time compensation payments. - Not really. Currently, two of our target specialists are undergoing internships. We urgently need pediatricians, functional diagnostic doctors, paramedics, nurses and other employees. - The problem of all enterprises and institutions is the sudden loss of workers - they got sick, went on vacation. How do they get out of the situation at the Sheksninskaya Central District Hospital?
- We use such a method of work as doctors having a second certificate. This system allows us to provide multifunctionality in the work of specialists, including replacing colleagues who are on vacation or on sick leave, expand the cognitive skills of doctors and provide medical care in several specialties at once. - You are right, these problems are topical for all regional centers of the region, but I would like to draw the attention of our citizens that when contacting paid clinics For patients, the first priority should be the question of the advisability of prescribing medical and other services to them, and then questions of price, conditions and quality. In our region, we are solving the problem of the lack of specialized specialists by attracting the necessary doctors to conduct consultations at the Central District Hospital within the framework of compulsory health insurance, that is, they provide free consultations to the population. In addition, I would like to tell Sheksna residents: before seeking paid medical services in private clinics, check with your attending physicians at free appointments whether there is a need and indications for additional examination and treatment.
Each patient has the right to check with the attending physician for information regarding this consultation or examination; perhaps, they can be provided to you within the framework of government guarantees. If we cannot provide some consultation or service in a district hospital, but it is indicated for you, then we are ready, moreover, we are obliged to provide referrals to other healthcare organizations in the region and country for specialized or high-tech medical care.
I would like to note that today there are two regional medical structures that our residents can contact in the direction of the Central District Hospital within the framework of compulsory medical insurance - regional No. 1 (Vologda) and regional No. 2 (Cherepovets). And to improve the quality of specialized medical care to the population of the Sheksninsky district, we organized field work within the framework of the state guarantee program for specialists from regional clinical hospitals of Vologda, Cherepovets and the regional children's clinical hospital. The visiting teams include a variety of doctors - an endocrinologist, a neurologist, a cardiologist, an ophthalmologist, a urologist, and pediatric specialists. Over 10 months, 5,551 people were consulted and examined, of which 3,872 were children. All savings will be used for the development of the hospital, including the purchase of modern diagnostic and medical equipment.
- Here we come to one of the main questions. What kind of equipment? And is it really possible that the tomograph promised several years ago will finally be installed? And you will be the same chief doctor of the Sheksninsky Central District Hospital who can do this!
- Yes, we plan to install it in the fourth quarter of 2016, in addition, within the specified time frame we will purchase additional medical equipment for laparoscopic operations. We hope that the auction will take place in November, and the equipment will be installed before the end of the year, and the operations necessary for the Sheksna residents will be carried out in our hospital, and they will no longer have to be sent to Cherepovets and Vologda. Moreover, we now have our own highly qualified specialist - the head of the surgical department, Boris Anatolyevich Minyukhin. In addition, equipment is being purchased to determine drug signs of intoxication, to identify drug addicts driving cars, who, like drunk drivers, pose a great threat to safety traffic. Today there are special requirements in this regard. We will have new audiometry equipment for examining Sheksna residents and conducting preventive examinations. Additionally, defibrillators and ECG machines were purchased for the emergency department and intensive care unit. New medical equipment will allow us to expand the range of services provided by our hospital, and this is only the first stage in the equipment.

I would also like to note that we managed to do this in addition to the fact that renovations were going on all year: the fifth floor of the ward building, where the children's department, Department of Neurology and Day Hospital. The renovation of the second floor of the ward building is almost complete. The premises are allocated for expanding the provision of outpatient services. The roof of the catering unit and the roof of the three-story ward building have been repaired; it is planned to repair the roof of the five-story building.
­ - Just unbelieveble! Where are the sources of funding, if it’s not a secret?
- In addition to the budget funds, which are determined by the Department of Health and Economics, which I spoke about above, Evgeniy Artemovich Bogomazov, the head of the Sheksninsky district, provides enormous assistance in solving tasks and problems; of course, these are the heads of enterprises and organizations of our district and region. A lot of people helped and continue to help: Stanislav Stanislavovich Berezin, deputy of the Legislative Assembly of the Vologda Region, head of the Sheksninsky LPUMG, A.A. and Yu.A. Atomyan (“AtAg”), directors of branches of the SOGAZ insurance group, ShKDP headed by A.I. Nakvasin. Tangible support comes from Sheksna CJSC, Sheksna-Vodokanal, Sheksna-Teploset, and district entrepreneurs. Help is not necessarily money, it helps with material and technical means, provision of transport services, provision of labor resources. Thanks to her, it was possible to repair the hospital's access roads. As you remember, there have been complaints about their condition for many years, severe patients on bumps and pits, tossed from side to side in ambulances (photo from the editorial archive). Now all the potholes are covered with asphalt chips. A large amount of work has been carried out for the benefit of our region together with the heads of the Sheksninsky correctional institutions, for which we are deeply grateful. The hospital is being revived, and help is coming from all sides, and we accept it, realizing that this is an advance of trust from the citizens of the region, in the hope of high-quality and affordable treatment.
- Alexander Vladimirovich, it becomes clear that you are using all possible legal means to increase the rating of the Sheksninsky Central District Hospital, and in other words, to preserve the health of the Sheksninsky residents. I know that, among other things, you are leading a great educational work in the team, starting with impatience, rudeness towards patients, indifference, and each such case is a hospital-scale emergency with all the ensuing consequences.
- There is no other way. First of all, everyone needs to start with themselves, with their attitude towards the chosen business. We work with people! My task is to return the trust and respect of Sheksna residents to the central regional hospital. Today there is a financial recovery in general and a recovery of the team from a purely human and spiritual point of view. Provided that we treat people adequately and with care, and they begin to treat us the same way. As of today, all pressing issues regarding hospitals have been resolved, and even in the clinic there are practically none. The coupon system started working. Our residents began to understand that if a patient has a coupon, he will definitely be seen at the specified time. In addition, thanks to the coupon system, the patient determines his own appointment time. If a person goes to the clinic from the village, he already knows that he can order a ticket in advance with the help of a local paramedic or general practitioner in order to get an appointment on time. If emergency or emergency assistance is needed, it is provided “on the fly,” as it should be. It all works and there are almost no complaints anymore.

It should be noted that the hospital increases the availability of medical care for the population of the region. They go to rural FAPs medical specialists and conduct consultations, including narrow specialists. Today, a dentist began his work at the Nifantovskaya outpatient clinic; additionally, a general practitioner, Evgeniy Yuryevich Ruzanov, is conducting appointments. Work goes on every day, systematically and without stopping. The main objectives of Sheksninsky healthcare are to preserve and strengthen the health of the population, ensure accessibility and further improve the quality of medical care, strengthen the material, technical and medical base and efficient use of industry resources. We understand that our path is thorny and multi-tasking, but we are confidently moving along this path towards achieving the goal set by the leadership, for the benefit of the population of the Sheksninsky district.

The Sheksnins have decided which doctor is closer to the people!

The regional competition “People's Doctor” in the Vologda region is being held for the fourth year in a row in accordance with the resolution of the government of the Vologda region and with the support of the United Russia party. During it, medical workers who enjoy special trust and respect of local residents are determined by popular vote. Based on the questionnaires filled out by residents of the Sheksninsky district, the voting results for the Sheksninsky central regional hospital were summed up, and the winners in each nomination were determined. We have already reported that among doctors the title of “People’s Doctor” was given to Dmitry Petrov. Ekaterina Petrova took second place, Olga Odochuk took third place. All three are young professionals.

But in the “People's Paramedic” nomination, only seasoned professionals were among the winners. Thus, for the third year in a row, Natalya Yuryevna Nosovenkova holds the palm. In 1991, a graduate of the Cherepovets Medical School, a resident of the city of metallurgists, arrived on assignment at the Syromyatkinsky FAP. Then she worked at the Sizemsky district hospital, and then for many years she gave to patients who needed ambulance. For the last four years, Natalya Yuryevna has been a senior paramedic at clinic No. 2, a local paramedic at a therapeutic area. Sheksnin residents trust their health to this woman in a medical gown, because she has enormous experience, knowledge, consolidated in practice, versatile - she will not get confused in any situation, since she has seen a lot during her work. But most importantly, she did not lose her compassion and mercy. Problems in the human body, physical pain did not become a routine for her. For this she is loved and respected.
Second place in this category was taken by her ambulance colleague Nadezhda Veniaminovna Stulikova (pictured above). Her medical experience dates back to 1978. The Churovskaya pioneer was an activist and “commanded” the sandruzina. Then her teachers prophesied her future - she must definitely become a medical worker.
- My profession is my calling. After receiving my diploma, I came to work at Churovsky FAP, then worked at Kvasyunensky, Rechno-Sosnovsky. On the ambulance service - since 1988. And while I have the strength, I will work, and also rush to call the sick, listen to them, measure their blood pressure, make diagnoses, give injections, help and save. They say there is great happiness when you go to work with joy and desire. I'm just that person. And my soul has not hardened over the years, I worry about every patient, and especially about the elderly. I feel sorry for the village grandparents, spiritual and physical pain I perceive everyone as my own.

Third place in this category went to another respected paramedic - Lyudmila Veniaminovna Vasilevskaya (pictured). Her medical work experience is already 41 years.

Doctor Pankov: “Life begins in the maternity hospital!”

Vladimir Alekseevich Pankov, a doctor at the Sheksninskaya Central District Hospital, became the winner of the regional competition “People’s Doctor”. Since 2004, he has been in charge of the obstetric department, where about 270 babies are born every year. Each of his working days begins with the maternity hospital, and almost every day is marked by the birth of a new person. But for the conversation, I suggested questions mostly not about work, but about personal life. What kind of son, husband, dad is he? What are your hobbies and how do you relax?

- Vladimir Alekseevich, I know that your father is a military man. Why did his son not want to follow in his footsteps, but decided to become a doctor?
- Today dad is a retired colonel. And my entire childhood was connected with the army, life on the road, I had to change seven schools during my studies. Just get used to your new place of residence, to the teachers, make friends with your classmates and everything will be new. I am the only son of my parents. I wanted to get out of this vicious circle, so I couldn’t even think about a military career. But I don’t know why exactly the doctor. Maybe my dad’s sister, my aunt, contributed, she worked as a paramedic. I wanted to be a surgeon. But in the first year I failed upon admission. I stayed to work at the institute and prepared intensively, so take-2 became successful. Unlearned. And then it was determined based on vacancies in the Gryazovets Central District Hospital. My family had settled in this town by this time. Surgeons were not required, but obstetricians and gynecologists were needed, so I completed an internship in this specialty. - You have three children. Who helped them come into being?
- At the birth of my first daughter, unfortunately, I was not there, but I did not trust the process of giving birth to both sons to anyone. Today Nadyushka is 12 years old, Egor is eight and Evgeniy is three.
- What reason made you leave Gryazovets and move to Sheksna?
- My wife worked as a midwife at the Gryazovets maternity hospital, when I came to work, we met and then got married. I wanted to live with my family, separately from my parents, but getting housing was unrealistic, as was buying it. Visiting doctors were given apartments, but since we had a place of registration, we were not entitled to such a benefit. So we moved to Sheksna, where we were given our own housing. Having settled here, we began to rarely come to our parents’ house. As I already said, I am the only child of my parents. They couldn’t stand it, sold the apartment and moved to Sheksna, buying their own home here. Dad and Mom became our support in everything, especially with the children. When a problem arose with the midwives at the maternity hospital, my wife even went to work early to support me. And grandmother Tanya took up the upbringing of her youngest grandson.

- I heard you are building a house?
- Yes! And our parents pushed us to take this step. They exchanged the apartment for a house in Nifantov and moved there, so to speak, closer to the ground. So we bought it nearby land plot and started construction. We hope to be able to move in by the fall.
- Do you build it yourself?
- If I had time, I would build it. But work takes a lot of effort and personal time. I'm just a helper for the builders. While on vacation, I restored an old bathhouse on the site. I dug up all the ground.
- How do you rest? Do you have a hobby?
- Rest for me is a change of activity. We have a dacha on Zaitsev. Although we planted the plot near the house under construction, we did not abandon the dacha. Moreover, we not only grow crops there, but also keep chickens and rabbits.

- Rabbits? Chickens? How do they winter there?
“We started breeding rabbits after our youngest son was diagnosed with an allergy to chicken and beef. Rabbit meat is a dietary product. At first we bought carcasses, and then we were offered a female rabbit who was expecting a litter. As a result, by the fall we raised a litter of rabbits, put the supplies in the freezer, and took the female to her parents for the winter. With the arrival of spring they started breeding again. Today we already have about 40 long-eared ones, and 10 hens with a rooster. We feed them all the waste from the garden, weeds - waste-free production it turns out. Everyone takes care of the animals. My wife got it specially driver license to be able to go to the country. Children are accustomed to work. They also have a house full of animals that need to be taken care of - a dog, a cat, two parrots, fish. For the winter we make a good supply of rabbit meat, providing ourselves with vegetables, berries and even fruits. Everything is environmentally friendly. What about hobbies? I enjoy restoring vintage motorcycles. I have already assembled an IZH-49 manufactured in 1956 using individual spare parts, and IZH-56 manufactured in 1961 is already on the move. I'm working on a new copy now. I repair, I collect for the soul.
­ - Who helps the children do their homework? What do they enjoy in their free time?
- Children's lessons are the wife's concern. But our Nadyushka is an independent girl. She does everything herself, and also helps her brother, a second-grader. The daughter is fond of basketball, Egor goes to the pool to swim, and also plays basketball. Both go to a tutor to study English in depth. For the youngest, kindergarten is enough for now.
- The maternity hospital is a “women’s kingdom”. Is it difficult to work in a team where your colleagues are women, and one of them is a wife, and the patients are only women.
- Women in the maternity hospital are unique specialists, women with masculine character, because they need to have an instant reaction, professional flair, the ability to independently make the right decisions, plus they need to be able to turn off emotions in work and at the same time show compassion, be sensitive and attentive. They are my team of professionals! It is not customary for us to say: “My working day is over.” If someone fails, another immediately takes his place, without complaints or offense, because it is impossible otherwise. Coupon system in the clinic of the Sheksninskaya Central District Hospital

In the clinic of the Sheksninskaya Central District Hospital, a coupon system has been introduced for appointments with specialized specialists - a surgeon, an ENT specialist, an ophthalmologist, a neurologist, and a dentist - in order to regulate the queue and make it easier for patients to visit medical institution. The administration did not reinvent the wheel; this is how they conducted receptions before, and this is how they conduct receptions in regional clinics.

Yukhno, the head of the outpatient department, now at the reception desk gives two coupons to patients who have applied for an appointment with a specialist at the Central District Hospital for the first time: a regular one for an appointment with a doctor and a coupon with the queue number and appointment time. In April 2016, a coupon system was introduced for appointments with dentists. Every day at the reception there are 9 coupons for initial dental appointments for treatment and 10 coupons for removal. From the beginning of May, a coupon system will be launched in the X-ray room, so do not be surprised when the doctor issues a coupon for a specific date and time along with a referral for a routine x-ray examination. The changes will also affect studies such as ultrasound. - Like my parents, I have three successors to the family. The youngest daughter is 1.5 years old, the middle one is a first-grader, and the son is 16 years old. He is a smart guy, there are no complaints about him and there is no need to demand success in his studies. I'm lucky! And he completely left the first-grader at the mercy of his mother, looking at her textbooks several times - “nowadays in school, 1st grade is worse than college.”
- It’s very interesting to know about your life outside the hospital. What are your interests and hobbies? What is your day off like?
- Despite the fact that I am with my family every day, there is still very little time for communication, unfortunately. Therefore, I try to devote myself completely to it on the weekends. In his youth he was fond of fishing, hunting, and now he has a family vacation at the dacha (Alexander Vladimirovich, smiling, shows traces of calluses on the palms of his hands that did not have time to fade after digging up the beds in the spring).

- Happiness is a reliable rear – a loving wife and children. Happiness is a job that you like and go to with pleasure. Happiness is when healthy and happy people surround you, and you do everything possible for this and give all your strength without reserve, and then rejoice with them in your joint victory over illness and disease.
- Is it possible to “turn your head off” from professional activities by closing the door of the Sheksninskaya Central District Hospital after a working day or week?
- I don’t turn off my head or my phone. I won’t have a full vacation or a weekend until I finally get the system working. But we are still far from ideal. We are constantly improving, moving forward in development and not standing still. It is no secret to colleagues that the chief doctor can come for an inspection in the middle of the night, on a weekend, or even just before the opening of the registry, in order to control the issuance of coupons and the attitude towards patients when making an appointment. Analyzing the work of the staff, I stand on the sidelines and observe the communication of my colleagues with the population. Systematic monitoring of the quality of medical care and communication with patients in hospital and clinic departments brings “fruits of joy,” namely, gratitude from the population in the organization of medical care, which they express directly during communication, and a decrease in the number of complaints about the medicine in our area. That is why I demand from my colleagues to keep me informed of all events in the district healthcare system, I do not leave a single complaint from the population unattended, because only joint work will give positive effect and will improve our activities, since there are always two sides to a “coin”, and all opinions must always be taken into account. We are intended to provide medical care to the residents of the area; we, doctors, cannot exist without patients, just as they cannot do without medical care. Our work must be built on the principles of mutual respect, understanding and compassion.
- What is the most difficult thing in your work today?
- I will put personnel first. They are the ones who decide everything. Today there is a good federal program “Zemsky Doctor”, when young specialists who come to work in the village are allocated allowances in the amount of 1 million rubles. This is significant support. Now the United Russia party is promoting the adoption of a similar program “Zemsky Paramedic,” which would solve the problem of securing paramedics in rural areas and rejuvenating the workforce. - What tasks do you set for yourself in the near future?
- Renovation of the second floor in order to expand the premises for providing outpatient care to the population. Reconstruction of the ambulance premises, close attention to the organization of medical care for the rural population and much, much more.
